I have a 97 CR-V with 36,000 on it. (That's not a typo, it's low mileage) I rarely take it on highways and when I tried last month it would not go over 30-35mph and the engine was not running well. I took the CR-V to a quick lube place for an oil change and they cleaned the fuel injectors and no better. They recommended I try a bottle of HEET which resolved the problem, it runs perfectly. About a tank later same problem came back, I put another bottle of HEET in and it runs fine.

What could I try? Should I change the fuel filter? Spark plug wires?

Take it for a good long drive. Let's burn through any impurities in the gas.

If you have ethanol free gas use that. With 150 miles a month I would encourage you never to fill the tank more than half way. Use of a fuel stabilizer is a good idea.
Try that approach for a few weeks and report back.
